Your Opportunity Integrated Home Care uses a strengths–based Client Centered Case Management practice framework. You will develop a care plan that supports the client and family to identify needs, focus on strengths, set achievable personal health and self‑care goals, and provide direct nursing care. In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization.
You will report to the Home Care Manager and be responsible for coordinating and providing quality care to clients in the community. You will provide nursing assessments, case coordination and direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment. The successful incumbent will demonstrate proficiency in personal and professional accountability, systems thinking, creative thinking, communicating and building relationships, organizational and time management skills, interpersonal skills and intermediate level of computer skills.
Description As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations while ensuring their safety and well‑being. You will utilize nursing processes through critical thinking, problem solving and decision making, teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.
You will provide safe, quality patient and family‑centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS‑HCP) certification.

Additional Required Qualifications 1 to 2 years of recent Home Care experience or an equivalent combination of education, training, and experience. Excellent organizational and time‑management skills.
Demonstrated critical thinking and decision‑making skills. Demonstrated assessment skills. Demonstrated ability to operate within an electronic health record.
Proven ability to work in an interdisciplinary team. Ability to work independently. Good negotiation and conflict resolution skills.
Ability to function effectively in a fast‑paced challenging environment. Travel required; possession of a valid driver’s license and access to a motor vehicle in good repair for work‑related purposes is a requirement. Required to operate a laptop computer and mobile phone, and perform manual handling (lifting, lowering, carrying, pushing and pulling).
For example, lifting of approximately 30 pounds in and out of a vehicle and carrying this weight up and down stairs throughout the day is common. Learning resources and training are provided to perform work safely. Physically able to perform all assigned tasks and duties in accordance with related legislation.
Preferred Qualifications Bachelor’s degree in nursing. Experience working with acute clients and/or long‑term clients with a focus on chronic disease management. Experience with mental health and/or addiction/harm reduction.
Experience providing case‑management services. Experience in Home Parenteral Therapy (HPTP), wound skin, ostomy care, palliative support and client teaching. #J-18808-Ljbffr
