Registered Nurse

Ontario Center, a specialized healthcare and rehabilitation provider in Ontario, NY, is seeking a dedicated Registered Nurse to join our Healthcare & Medical Services team. In this role, you will deliver person-centered, high-quality nursing care to patients with diverse medical and rehabilitative needs, ensuring safety, comfort, and dignity at every stage of treatment. You will perform comprehensive patient assessments, administer medications and treatments as ordered, monitor vital signs and clinical status, and document all care accurately in our electronic medical record system. Working collaboratively with physicians, therapists, and the interdisciplinary care team, you will help develop, implement, and regularly update individualized care plans aligned with each patient’s goals and level of function. The Registered Nurse will provide direct bedside care, manage IVs and wound care, support rehabilitation programs, and respond promptly to changes in condition, using sound clinical judgment to escalate concerns and advocate for patients. You will educate patients and families on diagnoses, medications, therapies, and self-care strategies to support safe transitions and optimal outcomes. Consistent with Ontario Center’s culture of excellence, integrity, and compliance, you will adhere to all infection control, regulatory, and safety standards while promoting a clean, organized, and healing environment. You will play a key role in fall prevention, pain management, and skin integrity initiatives and participate in quality improvement activities to enhance care processes. The RN will also coordinate and delegate tasks to LPNs and CNAs, providing clear direction, support, and follow-up to ensure continuity and consistency of care, and will communicate effectively during shift reports and interdisciplinary meetings.
