Bayshore is currently seeking a passionate and caring Visiting Nurse to join our exceptional care team in the community.Location: Home to home in the Greater Hamilton and Burlington Area. Driving within community is requiredPay: Pay per visitShifts: Monday through Friday, 7:00am to 3:00pm or 3:00pm to 11:00pm as assignedAlternating WeekendsStatutory holiday rotationOn-Call RotationWhat You’ll DoAs a Visiting Nurse, you’ll play a hands-on role delivering personalized high-quality care to your clients in their community. As a primary care planner to your clients, you have the autonomy and flexibility to provide a holistic approach to care while increasing your skills through our comprehensive training, mentorship and preceptorship programs.You will teach, delegate tasks and functions to different levels of clients and family members while maintaining our industry’s highest level of practice.You are the nursing care expert for your community.
With our efficient work schedules, you will be focused more on care time and less travel time. Common tasks can include, but are not limited to:Participate in on-call rotations as required (typically twice per month overnights)Conduct in-home assessments and deliver nursing care based on individualized care plansAdminister medications, wound care, IV therapy, palliative, pediatric care, and other clinical interventionsMonitor patient progress and update care plans accordinglyEducate patients and caregivers on disease management, medication adherence, and health promotion.Collaborate with interdisciplinary teams and task delegation to ensure continuity of careMaintain accurate and timely documentation using electronic health recordsWhy Choose Bayshore?Paid Time Off: Paid sick days, training and education days, including New Employee OrientationTop Industry Wages: Earn top wages for the critical work you do.
