Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: ASAP Duration: 13 weeks 40 hours per week Shift: 8 hours Employment Type: Travel Travel Physical Therapist (PT) – Alamosa, CO Assignment Start: ASAP Duration: 13 Weeks Schedule: 40 Hours/Week, 5x8s (Day Shift) Setting: Skilled Nursing Facility / Rehabilitation Responsibilities: Evaluate and assess patients to develop individualized treatment plans Implement therapeutic interventions to improve mobility, strength, balance, and functional independence Provide gait training, transfer training, and therapeutic exercise programs Document patient progress and maintain accurate clinical records Collaborate with physicians, nursing staff, and interdisciplinary team members Educate patients and caregivers on home exercise programs and injury prevention Requirements: Current Colorado PT license Graduate of an accredited Physical Therapy program (DPT or equivalent) Experience in skilled nursing or rehabilitation setting preferred Strong clinical decision-making and documentation skills Ability to work independently and as part of a multidisciplinary team Contact: HumanEdge Health HumanEdge Travel Healthcare Job ID 16408.
