Job Description

Provide dedicated nursing care as a Registered Nurse in community settings. Collaborate with clients and families to develop personalized care plans and support health goals effectively. This position requires a Registered Nurse to coordinate care plans while covering staff absences.

You’ll assess client needs, partner with interdisciplinary teams, and ensure ongoing modifications to services based on client responses. Flexibility and a valid driver's license for rural travel are essential for this role, which is based across multiple sites.
