Overview This exciting opportunity as Registered Nurse, Designated Float covers planned and unplanned staff absences. Integrated Home Care uses a strength–based Client Centered Case Management practice framework. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care.

In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization. You will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community. Job Responsibilities Provide nursing assessments, case coordination and direct care in partnership with short-term and long-term clients and their families in their own residence or an alternative care setting, as determined by the assessment.

Demonstrate proficiency in personal and professional accountability, systems thinking, creative thinking, communicating and building relationships, organizational and time management skills, interpersonal skills and intermediate level of computer skills. This role requires flexibility, adaptability and rural/remote travelling; possession of a valid drivers’ license and access to a motor vehicle in good repair for work-related purposes. This position works out of three sites: Northwest Centre 1, East Calgary Health Centre and South Calgary Health Centre.

Required Qualifications Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) certification.

If you have not practised as a Registered Nurse within the past five (5) years, completion of the following free courses is required prior to employment: CRNA Medication Management (Chapters 1‑4) and NextGenU Health Assessment Resources Certification. Additional Required Qualifications Minimum of 5 years of nursing experience with at least 2 years of community nursing experience including 1 year of recent Home Care nursing experience providing case management and nursing interventions. Ability to lift 30 lbs.

Operate a laptop computer and mobile phone, and perform manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) from Heart & Stroke Foundation within the last 12 months. Preferred Qualifications Bachelor’s degree in Nursing.

Experience working with acute clients and/or long term clients with a focus on chronic disease management. Experience providing case management, client teaching and professional nursing interventions including but not limited to Home Parenteral Therapy (HPT), wound care, ostomy care. Experience supporting vulnerable clients requiring palliative, mental health and/or addictions support.

Level 2 McMaster AHS Case Management education is an asset. Benefits Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable. #J-18808-Ljbffr
