Position Band 6 Glaucoma Nurse Specialist – Part Time An opportunity has arisen for a part time Band 6 Glaucoma Nurse Specialist within St.Paul's Ophthalmic Outpatients. This post is part of a Multi-Disciplinary Team dedicated to the assessment, monitoring and treatment of patients with Glaucoma. It will involve seeing Glaucoma patients within the outpatient department in nurse led clinics.
Key Responsibilities Act as a point of contact for existing and newly diagnosed glaucoma patients. Perform specialist assessment of patients' nursing needs, plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills. Lead clinical care by managing a patient caseload providing expert assessment, planning and evaluation.
Co‑ordinate and manage nurse/AHP‑led clinics within the specialty. Contribute towards a multi‑professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements. Establish and maintain effective communication with patients, carers and relatives.
Support patients and their families and ensure that they receive the required information to enable them to self‑manage, lead or participate in their care delivery. Recognise changes in patients’ condition that require the intervention of others and refer on as appropriate. Support, assist and encourage the multi‑disciplinary team in resolving patient problems, by the provision of new and innovative models of case management.
Lead the specialist nursing contribution to the service, working in partnership with lead clinicians, General Managers and the Matron to ensure the delivery of high standards of patient care. Work with the Matron to promote the development of nurses/AHPs undertaking the CNO ‘Ten Key Roles’ as considered appropriate (e.g. prescribing). Will deputise for the Band 7 manager.
Qualifications and Experience At least 2 years of experience working within an Ophthalmic setting. Possess a relevant Ophthalmic Nurse Specialist qualification. ANEX 21 (training post) may be considered to gain the required competencies. (75% of the Band 6 will be paid until completion of competencies). #J-18808-Ljbffr
