Community Lead Nurse

What You'll Be Doing You will be part of a compassionate team committed to our clients’ wellbeing. Write support plans, risk assessments and have knowledge of ACP; undertake routine tasks and activities as directed or delegated to facilitate wellbeing, dignity and treatment of patients. Deliver prescribed care to own allocated clients.

Assess, plan and deliver nursing care with the client and their carer/family, taking account of clients’ choice and wishes and acting as the client’s named nurse. Accurately observe and engage with clients to assess their needs and evaluate their progress. Monitor and review nursing support plans.

Evaluate and respond to changes in patient conditions and needs. Attend MDT meetings and confidently discuss own clients. Liaise with health professionals regarding clients’ needs.

Have strong and proficient computer skills. Travel for services along the M5 corridor. What You'll Bring to the Role Our work is rewarding but can be challenging, so resilience is key.

No two days are the same. Your natural empathy will help you support others through both the highs and lows. Skills & Qualifications Required Qualified Registered Nurse (RGN) with 6-12 months of post‑registration experience.

RGN qualification highly desirable. Solid knowledge of NMC guidelines, requests, and professional practices, with evidence of continuing professional development. Positive attitude and a strong commitment to change, improvement, and quality.
