Registered Nurse (RN)- UAS Home Health (Hybrid)

Job DescriptionJob Description WCS Healthcare is partnering with a respected Home Health Agency to hire experienced Registered Nurses (RNs) with UAS Assessment experience for multiple openings in Jackson Heights, Queens.

Position Summary

As a UAS Assessment Nurse, you will conduct comprehensive in-home assessments utilizing the UAS-NY system to evaluate patient needs and determine eligibility for long-term care and home-based services.

This role offers a balanced schedule with time spent both in the field and working remotely.

Key Responsibilities

• Conduct UAS-NY assessments in members' homes throughout the Jackson Heights and surrounding Queens territory
• Evaluate members' physical, functional, cognitive, behavioral, and psychosocial needs
• Complete accurate and timely UAS documentation and assessment reports
• Determine eligibility and level-of-care recommendations for community-based services
• Collaborate with physicians, care managers, social workers, caregivers, and interdisciplinary teams
• Educate patients and families regarding available services and healthcare resources
• Maintain compliance with agency policies, documentation standards, and New York State regulations
• Manage assigned caseload and meet productivity expectations

QualificationsRequired

Active New York State Registered Nurse (RN) License

Current UAS-NY Certification

Previous UAS Assessment experience

Strong clinical assessment and documentation skills

Excellent communication, organizational, and time-management abilities

Preferred

• Home Care experience
• MLTC (Managed Long-Term Care) experience
• Case Management experience
• Community Health experience

Schedule

Full-Time

Monday-Friday

9:00 AM-5:00 PM

Hybrid Schedule

3 Days Conducting Assessments in the Field

2 Days Working Remotely

No Weekends

Territory

Jackson Heights, Queens

Candidates will be assigned a localized territory to help minimize travel and maximize efficiency.
