Title: Nurse Practitioner (Primary Care / Nursing Home)
Location: Bronx, NY
Salary: $135,000–$150,000/year (DOE)
Job Type: Full-Time Permanent Direct Hire
Overview:
Seeking a Nurse Practitioner to provide comprehensive primary and specialty care in a collaborative clinical setting. Responsibilities include patient assessment, diagnosis, treatment planning, preventive care, and care coordination.
Responsibilities:
- Perform histories and physical exams
- Diagnose and manage acute/chronic conditions
- Develop individualized care plans
- Order and interpret labs and diagnostic tests
- Prescribe medications and administer treatments/vaccinations
- Provide preventive care counseling
- Maintain accurate EMR documentation
- Coordinate referrals and specialty care
Requirements:
- Master’s degree from an accredited NP program (Family or Adult/Gerontology)
- Active New York State NP license
- AANP or ANCC certification
Preferred:
- Specialty certification (e.g., ONCC)
- EMR/EHR experience
- Bilingual Spanish
