Position Details We are seeking an Admissions RN to provide information about services to potential agency patients and admit patients to the agency's total care category of service in coordination with the Clinical Team Manager(s). Responsibilities Screen and process inquiries for admission to MHHC's total care category.
Determine patient’s eligibility for hospice care. Make telephone contact and/or conduct assessment visits with potential hospice patients to provide information on agency services and/or assess patient status. Assist with implementation and coordination of services required for discharge of patients from hospitals into the agency's program of care.
Schedule Friday, Saturday and Sunday; 11:00 am – 7:00 pm. Location San Mateo County & Palo Alto (In‑Patient Unit at Stanford Hospital). Education & Experience One year experience as a professional nurse in the past three years or a Bachelor’s degree in nursing from an accredited School of Nursing.
Experience in acute care, home health and/or hospice setting preferred. Certifications & Licenses Current license as a registered professional nurse in the State of California. Public Health Nursing certification highly desirable.
