The Physical Therapist (PT) evaluates, plans, and implements individualized treatment programs to improve mobility, strength, and overall functional independence. KEY RESPONSIBILITIES Evaluate patient conditions and develop personalized treatment plans Implement therapeutic exercises and rehabilitation techniques Monitor patient progress and adjust treatment plans as needed Educate patients and families on recovery and mobility strategies Maintain accurate and timely clinical documentation Collaborate with interdisciplinary healthcare teams Ensure compliance with facility policies and safety standards REQUIREMENTS Active Physical Therapist (PT) license (state-specific) Graduate of an accredited Physical Therapy program Previous rehab or skilled nursing experience preferred Strong clinical, communication, and documentation skills
