Job Description
Enhance patient care as a Specialized Nurse focusing on wounds, ostomies, and continence. Collaborate with an interprofessional team to provide comprehensive assessments and care management. As a vital part of the Wound Ostomy Continence (WOC) Team, you will deliver holistic assessments to individuals with ostomy and wound-related concerns.
This role includes treatment planning, evaluations in collaboration with team members, and providing education to build capacity within the organization. You'll work toward optimizing client outcomes through mentorship and participation in quality monitoring initiatives. Key Responsibilities: • Conduct detailed holistic assessments for clients • Collaborate with the team to create care plans • Evaluate client outcomes and adjust care plans accordingly • Provide education and mentorship for improved knowledge • Disseminate best practices among care providers Requirements: • Completed NSWOCC WOC-EP program • Active nursing registration in Ontario • Commitment to advancing care in the field • CNA certification preferred • Excellent communication and teaching skills Utilize your expertise to enhance wound and ostomy care and contribute significantly to patient outcomes. #J-18808-Ljbffr
