How you'll make a difference

As a Social Work Assistant, you will be working directly with disabled children and young people, their families and other networks. To ensure they have the best possible outcomes, you will provide support and inclusion to enable them to reach their full potential.

You will be part of the larger 0-25 social care service and work within the 16-25 team.


What you will be doing

  • You will be working with young adults who require social care services and need support to help plan for the next stage in their life this will involve assessing and reviewing cases, care plans or re enablement programmes, making recommendations for changes or adjustments as necessary.
  • You will be working in a strength-based conversational model of practice (3 Conversations) and working in close partnership with our Pathway to Independence team.
  • It will be key for you to work and liaise with the community, volunteers and external contacts in order to gain wider and more inclusive support for clients.
  • Working alongside our Social Workers, you will help families understand that their young people are now adults and are able to make their own choices for greater independence. Navigating the transition process into adulthood with different professionals and legislation involved.
  • You will be required to travel widely within the authority, so you must either have a current driving license and provide a car or have access to appropriate means of travel.

What we need from you

  • A relevant qualification (NVQ Level 3 in health or social care) or experience of statutory or voluntary sector work in the health or social care field.
  • Youll have the ability to gather information from a variety of sources and follow assessment processes, maintaining timely and accurate records.
  • Youll understand the needs of service users and have good interpersonal skills with the ability to work alongside service users, other professionals and service
  • providers in a way that promotes independence and personal choice.
  • Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.

  • You will be required to travel widely within the authority so you must have a full, valid driving licence and provide a car or have access to an appropriate means of travel.
  • Business insurance will need to be added to your current car insurance policy.
