Job Description Job Description Licensed Vocational Nurse (LVN) – Home Health ???? Location: San Diego ???? Pay: $60–$70 per visit About the Role Guardian Angel Home Care is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our growing home health team.

In this role, you will provide high-quality, patient-centered care in the comfort of patients’ homes while working alongside a supportive interdisciplinary team. This is an excellent opportunity for LVNs who enjoy autonomy, flexible scheduling, and building meaningful relationships with patients. What We Offer Competitive per-visit pay ($60–$70 per visit) Flexible scheduling Electronic charting (Homecare Homebase – HCHB) Supportive clinical leadership Opportunities for growth and advancement Responsibilities Provide skilled nursing care in patients’ homes Monitor patient condition, vital signs, and overall health status Administer medications and treatments as ordered by the physician Accurately document all visits and care provided Communicate changes in patient condition to the RN/physician Educate patients and families on care plans, medications, and disease management Ensure compliance with all state, federal, and agency regulations Collaborate with the interdisciplinary team to ensure optimal patient outcomes Requirements Active California LVN license in good standing Graduate of an accredited practical nursing program BLS certification Strong clinical, communication, and documentation skills Ability to work independently in a home health setting Preferred Qualifications Home health or hospice experience Experience with Homecare Homebase (HCHB) or other EMR systems IV therapy, wound care, or other specialty certifications Bilingual is a plus Why Join Us?
