Position Summary: Reporting to the Clinical Nurse Director, this position is responsible for planning, directing, coordinating and providing high quality, individualized patient care using the components of the nursing process. Performs duties in a manner that provides for the physical, psycho-social and spiritual needs of the critically ill patient includes completing initial assessment in a timely manner and the ongoing reassessments when warranted by the patient's condition. Incorporates cultural and ethnic factors into assessments/reassessments; correctly differentiating between normal and abnormal findings; and incorporates the functional/developmental age into assessment/reassessment for the adolescent through geriatric patient population.

Uses hospital and regulatory standards of care, the nurse demonstrates sound clinical judgment in the development, implementation, and evaluation of the nursing aspects of the trans-disciplinary care plan. In conjunction with the patient/family, the nurse develops an individualized patient care plan and determines interventions based on assessment/reassessment findings. The nurse evaluates patient outcomes on an ongoing basis, establishes priorities for patient care services as evidenced by modification of the care plan, if necessary, and demonstrates the ability to accurately document patient care activities in the medical record.

Utilizes a case management approach, the nurse plans, guides, directs, coordinates and collaboratively incorporates the overall aspects of patient care across the continuum by effectively communicating with physicians, staff and other disciplines. Safely and competently administers pharmacotherapeutics adhering to medication policies, procedures, practices and standards, and monitors the resultant outcomes based upon the patient's response to treatment. Assesses and responds appropriately to ensure patient safety based on the physical environment, equipment and the patient's behavior.

Demonstrates the ability to provide care in emergencies and follows established emergency procedures and practices. Communicates and follows the organizational chain of command for notification of patient care/service issues, when appropriate. Verbalizes and demonstrates the ability to obtain and interpret cardiac rhythms and the treatment of arrhythmia.

Maintains knowledge of, and demonstrates the ability to identify therapeutic patient care issues/problems and determines measures and interventions to promote, maintain and/or restore health for the adolescent through geriatric patient population as described in the Nursing Skills Inventory. The nurse also provides appropriate care through the healthcare continuum including the end of life. Assures that the rights of all patients are respected and maintained by allowing for privacy and dignity in the provision of patient care.

Determines the presence and type of Advance Directive, if applicable. The nurse ensures patient and/or family involvement in the decision-making process as it relates to patient care, end of life and ethical issues. The nurse is aware of the reporting responsibilities and reports suspected cases of abuse or neglect relative to the adolescent through geriatric patient population as appropriate.

Identifies patient/family educational needs; initiates and provides individualized teaching which considers the functional/developmental age and the needs of the adult patient population. The nurse evaluates and documents the effectiveness of the patient/family teaching through assessment of their comprehension of the instruction and demonstration of appropriate responses. Demonstrates knowledge of age specific care, includes but not limited to, identifying the need for additional safety measures, physiological normal values/readings; and assessing skin integrity, behavior, motor skills and/or activities that place patients at risk as well as communicates effectively in a clear, concise, understandable manner.

Consistently ensures Core measures for Department are implemented and documented. Consistently participates in the unit and hospital specific action plans to improve HCAHPS scores. Is able to use AIDET in all sittings.

Ensures completion of hourly rounding and updates white boards in a timely manner. Understands is individually accountable for achieving Core Measures, HCAHPS and other VBP goals Professional Requirements Reports to work on time and as scheduled, completes work within designated time. Uses computerized time clock correctly.

Attends staff meetings and actively participates, reads and returns all staff meeting minutes. Actively participates in performance improvement and continuous quality improvement (CQI) activities. Demonstrates an understanding of the procedures for reporting and responding to facility emergencies such as fire, and disruption in utilities and hazardous materials spills.

Demonstrates knowledge of equipment capabilities, limitations, and appropriate/special applications. Demonstrates the ability to access and use computer systems. Communicates with physicians, administrative staff, co-workers, and patients/families in a respectful and caring manner.

Demonstrates a positive professional image; performs responsibilities in a professional manner; demonstrates flexibility in the provision of patient care/service; utilizes a collaborative approach with co-workers. Demonstrates flexibility in the work setting with changing assignments, varying staffing levels and patient care environments, and completes work on time. Consistently demonstrates critical thinking and problem solving skills.

Assists department management in the resolution and evaluation of problem situations. Assumes other duties as required. Behaviors Adheres to dress code, appearance is neat and clean.

Completes education requirements in a timely manner. Maintains regulatory requirements, including all federal, state, local regulations and accrediting organization standards. Wears identification while on duty and ensures it is visible at all times.

Maintains patient confidentiality at all times. Can state the mission, vision and values of the facility. Complies with the Medical Centers policies and procedures, including timekeeping, attendance, the Standards of Behavior and the Code of Conduct.

Maintains required applicable licensure, certification, educational and in-service requirements. Displays positive attitude through words and actions. Displays willingness to adapt to new ideas and contributes to effecting progress.

Demonstrates awareness of who the customers are (internal/external) and supports the company's goal and objectives in a positive manner. JOB QUALIFICATIONS Minimum Education (Indicate minimum education or degree required.) Graduate of an RN Training Program Completion of Basic EKG course Preferred Education (Indicate preferred education or degree required.) Completion of a Critical Care course preferred. Minimum Work Experience and Qualifications (Indicate minimum years of job experience, skills or abilities required for the job.) Ability to communicate effectively verbally and in writing.

Must be able to work in a union environment. Preferred Work Experience and Qualifications (Indicate preferred years of job experience, skills or abilities required for the job.) Minimum one year recent experience in an acute care setting preferred. Two years' experience in a critical care or telemetry setting preferred.
