Schedule Day Shift: 5x8 Hours (07:30 - 16:00) Job Description We are seeking a dedicated Acute Care Case Manager with experience in discharge planning and care coordination. The ideal candidate will be responsible for managing and facilitating care transitions for patients within a Level II Trauma Center.
Responsibilities Coordinate discharge planning and care for patients across various hospital units. Work collaboratively within an interdisciplinary team to ensure patient needs are met. Float between multiple hospital areas, including Colby and Pacific Campuses.
Support patients in understanding their care plans and assist with post-discharge needs. Required Experience / Certifications / Licensure Minimum of 1 year of acute care experience is required. Certifications: BLS, RN Licensure.
Experience in discharge planning and care coordination. Strong preference for candidates with diverse case management experience, including Med Surg and EDC. Why ARMStaffing?
