Remote/Hyrid Working Nurse Manager – Community Complex Care Salary £48,000 Basingstoke, Reading, Oxford Hybrid: 3 days community-based | 1 day from home | 1 day in office UK Driving Licence & Own Car Required Have you got what it takes to succeed The following information should be read carefully by all candidates. Sentra Care are working in partnership with our client, a recognised leader in the Community Complex Care sector , to recruit an experienced Clinical Lead to support adults and children with complex healthcare needs in the community. This is a highly autonomous, clinically focused role offering a true hybrid working model, with the opportunity to make a meaningful impact on client outcomes while maintaining a healthy work–life balance.

The Role you will be responsible for managing a community-based caseload across Basingstoke, Reading and Oxford , ensuring the delivery of safe, high-quality, nurse-led care. The role will consist of: 3 days per week in the community , completing client assessments, staff training, touchpoints, reviews, and clinical oversight 1 days per week working from home for care planning, governance, and MDT coordination Office Days for team meetings and training. You will work closely with Care Coordinators, operational teams, and senior clinical leadership to ensure robust, client-centred care delivery.

Key Responsibilities Leadership & Collaboration Report into the Regional Lead Nurse and work alongside Practice Educators, and Care Coordinators Build strong relationships with clients, families, community teams, and specialist clinicians Work with central resourcing teams to ensure a skilled and consistent care workforce Service Delivery, Quality & Governance Create, review, and update person-centred care plans and risk assessments Provide visible, hands-on clinical support in the community Conduct client reviews at least every 8 weeks Participate in the nurse on-call rota Escalate clinical and safeguarding concerns appropriately Support complaint resolution and clinical investigations Ensure compliance with clinical policies and regulatory standards xwzovoh Essential Requirements NMC Registered Nurse Background in ICU/ITU/HDU or complex continuing healthcare - MUST HAVE Recent Tracheostomy & Ventilation exp - within last 6 months Right to Work in UK - They cant offer Skilled worker visas at this stage Must be based in Either Hampshire, Reading, Oxford or surrounding areas Post-qualification experience in acute clinical care Experience mentoring, supervising, and developing staff Strong communication, analytical, and organisational skills Ability to work autonomously and manage a community caseload Full UK driving licence and access to your own vehicle Remote working/work at home options are available for this role.
