Go back University Hospitals of Leicester NHS Trust Registered Nursing Associate The closing date is 08 May 2026 A Community Nursing Associate (Band 4) working with a diabetes focus supports the delivery of high-quality, holistic nursing care to people living with diabetes within community settings such as primary care networks (PCNs), outpatient clinics, patients' homes and care homes. Main duties of the job To provide holistic, person-centred nursing care that addresses physical, psychological, social, and public health needs. To support diabetes management and prevention, ensuring patients receive appropriate monitoring, education, and access to essential diabetes care.
To work as part of a multi-disciplinary community team, contributing to health promotion, early intervention, and ongoing support for long-term conditions. Monitor patients with diabetes, ensuring access to diabetes care essentials (e.g., blood glucose monitoring, CGM, injectable therapy, foot checks etc.). Provide lifestyle and self-management education, promoting diabetes prevention and health improvement.
Conduct home visits to support patients with diabetes when required. Job responsibilities Provide holistic, person-centred nursing care to patients in their own homes or clinic settings. Work within Nursing Associate parameters of practice, ensuring care aligns with the NHS 6 Cs.
Support Diabetes Management & Prevention Deliver care focused on people with diabetes, ensuring they are appropriately monitored and receive the diabetes care essentials. Participate in clinics and services that promote early intervention, self-management, and healthy lifestyle behaviours. Work Under Delegation of a Registered Nurse Carry out delegated clinical and care tasks (e.g., observations, wound care, specimen collection) to an agreed standard and competency.
Follow individualised care plans and elevate concerns appropriately. Manage and Prioritise a Delegated Caseload Proactively organise and prioritise patient appointments and home visits within your competency. Make autonomous decisions about patient care within agreed boundaries, seeking support when needed.
Contribute to MultiDisciplinary Team Working Collaborate with diabetes specialist nurses, community nurses, PCN colleagues, GPs, and allied health professionals. Provide health education, screenings, and advice aimed at diabetes prevention and improving long-term condition management. Promote independence and self-management among patients.
Record Keeping & Administrative Duties Maintain comprehensive, accurate, contemporaneous electronic and written patient records. Ensure templates (e.g., QOF-related) are completed correctly. Training & Qualifications Be a Registered Nursing Associate.
Level 2 / GCSE or equivalent English and maths or Functional Skills. Placement experience working within the 4 fields of nursing. Experience Understands and acts in line with the NMC professional standards for practice contained with The Code and NMC Standards of Proficiency.
Understands the scope of the role of the Nursing Associate in the context of nursing and interdisciplinary team and the organisation and how the role may contribute to service development. Knowledge of when to seek advice and escalation to the appropriate professional for expert help and advice. Ability to participate in reflective practice and understand the requirements for NMC Revalidation.
Understands the importance of following procedures and treatment plans. Demonstrates knowledge of evidence based practice. Demonstrates knowledge of clinical governance and clinical effectiveness.
Demonstrates an understanding of current issues relating to the NHS. Skills Ability to deliver patient-centred care. Ability to recognise when escalation to a registered professional is required (e.g., registered nurse / doctor).
Ability to take part in reflective practice and clinical supervision activities. Ability to organise and prioritise own delegated workload. Ability to deal with non‑routine and unpredictable nature of workload and individual patient/ service user contact.
Ability to communicate effectively (written, verbal and non‑verbal communication) with patients/relatives and carers and all members of the multi‑disciplinary team. Ability to develop effective and appropriate relationships with people, their families, carers and colleagues. Ability to support, supervise, assess and act as a role‑model to nursing associate students, other learners and health care support workers as required within the clinical setting.

Computer literate, good working knowledge of Microsoft Office. Ability to teach, assess and supervise others. Ability to use a variety of presentations skills to engage with individuals with a multitude of learning styles.
Be able to work at multiple locations and have own transport.
