Description Job Opportunity: Registered Nurse - Transplant Facility: Sutter Health CPMC Van Ness Campus Location: San Francisco, CA Employment Type: Travel/Contract Shift: Day (5x8 Hours) 8:00 15:30 Start Date: 5/19/2025 Job Duration: 13 weeks Compensation: Rate Type: Hourly Over Time: 60 total hours in 1 week: 1.25% Double Time: None On-Call: 7$ Call Back: 1.35% Holiday Pay: 1.25% Additional Information: Job Description: • Coordinates all aspects of care (inpatient and/or outpatient) - per program assignment, including but not limited to record review, medication refills and/or prior authorizations; patient education, review of test/procedure/lab results • Makes referral for medically necessary services, consults, equipment and supplies. • Functions as a liaison between administrative, clinical staff and physicians within the transplant department, as well as other departments involved with the care of patients. CARE PROVISION: • Assists and supports patient discharge planning as needed • Provides patient education regarding care and medications • Provides assistance to patients and/or physicians in clinic and on hospital floors, as needed. • Carries out patient care orders as directed by physician and in accordance with hospital policy. • Assists with patient admissions and discharges as required Requirements: BLS CA License Bachelor's, Shift Details: M-F, 5x8s no weekends or holidays
