Position Details

Position Title: Senior Long-Term Service Care Manager (RN)

Location: Remote Texas - Member-facing

Duration: 6-Month Contract (possibility of conversion)

Schedule: Monday–Friday, 8:00 AM–5:00 PM

Position Summary

We are seeking an experienced RN to serve as a Senior Long-Term Service Care Manager. In this role, you will oversee the assessment, coordination, and delivery of care services for complex, high-acuity members with medical and physical health needs. You will develop personalized care plans, ensure members receive appropriate services, and act as a key advocate and resource for members and their families/caregivers.

Key Responsibilities

  • Develop, implement, and update long-term care plans for high-risk members.
  • Coordinate care between members, families, caregivers, providers, and community resources.
  • Monitor member status, identify changes in condition, and adjust care plans as needed.
  • Facilitate education for members and caregivers regarding conditions, benefits, and available resources.
  • Conduct home or site visits (monthly or more, as required).
  • Ensure compliance with state, federal, and clinical guidelines through accurate documentation.
  • Partner with leadership to improve service quality and outcomes.
  • May serve as a preceptor/mentor for new clinical hires, supporting onboarding and training.

Qualifications

  • Education: Graduate from an accredited School of Nursing required; BSN preferred.
  • Experience: Minimum 2–3 years of pediatric nursing experience required; 4–6 years total related experience preferred. Case management and Medicaid/home- or community-based care experience strongly preferred.
  • Licensure/Certification: Active RN license (Texas or compact state). RUG/PDPM certification required.
  • Skills: Strong care coordination, case management, and patient advocacy abilities.
