Registered Nurse (RN) - Case Manager - Home Health Join to apply for the Registered Nurse (RN) - Case Manager - Home Health role at Comfort Home Care . Overview We currently have an opening for a Registered Nurse (RN) in the Cambridge area (Monday through Friday - AM shift). Our nurses help psychiatric and home health patients remain independent, avoid unnecessary hospitalizations, and receive compassionate care in the home setting. Responsibilities Assess and Adapt: Evaluate the patient’s current condition, medical needs, and overall well-being at each visit. Create and Modify Care Plans: Actively shape treatment plans based on patient needs and changing conditions.

Teach and Support: Provide medication guidance for psychiatric patients and wound care for post-op patients to help them stay independent at home. Communicate and Coordinate: Keep providers, families, and care teams informed to ensure timely and appropriate care. Document and Reflect: Submit visit notes from home to ensure accurate records for compliance and continuity of care.

What We're Looking For (Qualifications) Active Registered Nurse (RN) license in Massachusetts. One year of nursing experience (home health or psych preferred). A drive to provide compassionate, patient-centered care.
