Job Title: Registered Nurse (RN) Case Manager Location: Garland, TX Employment Type: Full-Time, Permanent Overview: Our home health team is seeking a Registered Nurse Case Manager to deliver personalized care in patients homes. This role focuses on managing care plans, coordinating services, and supporting patients and families to achieve optimal outcomes in a structured weekday schedule.
