The Bloom Clinic is committed to providing treatment, care, and support to individuals living with, affected by, or at risk of Hepatitis C (HCV) and/or HIV across the Region of Peel. Bloom Clinic is committed to providing community-based testing, treatment, and support to people who are experiencing homelessness, people who use drugs, people who are incarcerated or otherwise involved in the criminal justice system, and Indigenous community members.

The Registered Nurse is a member of a mobile and interdisciplinary team delivering itinerant services throughout the Region of Peel to people living with or at risk for acquiring Hepatitis C (HCV) and/or HIV, practicing to full scope in accordance with College of Nurses of Ontario (CNO) standards, organizational policies, and approved medical directives. Responsibilities Practice to full scope as a Registered Nurse in accordance with College of Nurses of Ontario (CNO) standards, organizational policies, and approved medical directives, within an interdisciplinary and community-based outreach model Provide Hepatitis C, HIV and STI screening and counselling at various outreach locations across Peel Region with an emphasis on community settings serving priority populations Communicate diagnoses in collaboration with the most responsible primary care practitioner (NP), identify treatment goals, develop treatment plans, and provide support throughout the treatment process for clients living with Hepatitis C and HIV. Deliver comprehensive sexual health services including education and counselling, STI testing, treatment and management, pre-exposure prophylaxis (PrEP) and Post-ExposureProphylaxis (PEP) assessment, initiation, and ongoing clinical monitoring in accordance with approved organizational medical directives.

Conduct screening and refer clients to appropriate community agencies for mental health and addiction programs, housing services and other supportive services Provide education and counselling pertaining to Hepatitis C and HIV, PEP, PrEP, liver health, safer sex and drug use, overdose prevention, and wound care Order and interpret routine and specialized laboratory tests and imaging in accordance with medical directives and best practice guidelines Provide opioid overdose response as needed including naloxone administration and BLS/CPR Act as Program Lead for HIV and Hep C point of care testing; including ordering HIV and Hep C testing kits, maintaining inventory, performing quality control testing, and organizing team proficiency testing and ongoing training. Maintain and manage vaccine storage; ordering regular vaccine supply, monitoring the fridge temperature, and addressing and managing cold chain incidences. Determine financial eligibility and support with application forms for Hepatitis C, HIV, PEP and PrEP drug coverage Support the development and implementation of clinical workflows for HIV and Hepatitis C care Deliver clinical practice that is evidence-based and guided by medical directives, organizational policy, and best practice guidelines Lead and participate in regular case management meetings to identify and address complex health and psychosocial client needs Establish and maintain community partnerships to expand outreach services, increase access to health and social services and foster linkages to care, mobilize resources, and address social determinants of health to meet the complex needs of clients Maintain up to date knowledge of community resources Participate in the overall activities of the health centre, e.g. in service training, quality improvement, accreditation, committees, special events and shared staff responsibilities Actively participate in WellFort’s commitment to equity, inclusion, and culturally safe care for all staff and community members #J-18808-Ljbffr
