Clinical Care - Registered Nurse - Support at Home

The Role
Reporting into the Clinical Governance and Quality Manager, this role is responsible for delivering clinically safe, person centred nursing services to older people receiving government funded home care, primarily via telehealth. This is a billable clinical role with meaningful variety, encompassing telehealth assessments, medication management, wound management oversight, restorative care coordination, and end of life care planning. Additionally, you will:
- Conduct structured telehealth clinical assessments, document findings, and provide clear care plan and budget recommendations
- Manage government systems to make and update Support at Home funding applications
- Assess medication related risks and liaise with GPs and pharmacists
- Liaise with wound care specialists, triage via telehealth, develop and adjust wound management plans, and elevate for in person reviews
- Support time limited, goal based restorative care plans to build client independence, coordinating with allied health providers and monitoring outcomes
You are a clinically experienced Registered Nurse with a genuine passion for aged care and community health, comfortable working proactively in a telehealth environment. You bring strong digital literacy, excellent documentation practices, and the ability to collaborate effectively across multidisciplinary teams. Additionally, you possess:
- Current AHPRA registration as a Registered Nurse (unrestricted)
- Minimum three years post registration experience in community, aged care, general practice, chronic disease or similar
- Experience and knowledge of Support at Home or Home Care is ideal
- High competence with telehealth platforms, clinical systems, and digital documentation
- Proficiency in Microsoft Office, including Word, PowerPoint, and Excel
- Current National Police Check
