Base pay range $95,000.00/yr - $100,000.00/yr Role: Registered Nurse Organization: Edenbridge PACE at Skyland Location: Washington, DC SE Status: Full-time Mission: To allow frail elderly people to age in the location of their choosing and continue to lead connected, meaningful lives Job Summary Under the direct supervision of the Medical Director, provides skilled nursing care to participants.
This position is responsible for working directly with the Interdisciplinary Team and participant family members or caregivers to achieve the goals of the care plan. Communicates to the family or caregiver and provides appropriate patient education. Main Responsibilities Must maintain calm and professional demeanor.
Performs blood draws and other laboratory testing, medical management, IV therapies, wound care, etc. as directed by medical providers. Documents test results and examinations in the EMR. Implements orders written by medical providers.
Provides medication reconciliation and communicates with facilities and the pharmacy. Communicates effectively to the IDT and to family and caregivers, including providing instructions regarding medications and treatment. Performs home visits to participants to provide medical care in the home as indicated.
Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians Follows all PACE4DC Policies and Procedures and OSHA safety guidelines. Maintains the confidentiality of all company procedures, results, and information about participants, clients, or families. Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions.
Maintains professional and courteous behavior on the job. Ensures customer satisfaction. Will support the success of the entire healthcare team.
Maintain a safe working environment. Follows PACE4DC Safety Policies and Procedures. Participates in continuing education classes and any required staff and training meetings.
Maintains professional affiliations and any required certifications. Performs other duties as required or requested in a positive and helpful manner to ensure a smooth-running work area Additional responsibilities will be added at managers discretion, as career growth opportunities and responsibilities are constantly arising in our flexible and start-up environment QULIFICATIONS Graduate of an accredited Registered Nurse Program and licensed as a professional in the District of Columbia is required. One year of health care experience in a hospital, long-term or home health care setting with 1 year of experience with a frail or elderly population is required.
Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers. Must submit to and pass a pre-employment health screening and criminal background check. Must have a current DC Driver’s License and driving record must be in compliance with Company policy.
Must maintain current automobile insurance. Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records. Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public.
