Job Overview

We are seeking a Registered Nurse Case Manager to provide compassionate nursing care and medical support to seniors. The role involves administering medications, monitoring vital signs, and collaborating with a multidisciplinary team to ensure coordinated care. You will educate patients and their families on health management and self‑care, and maintain accurate documentation in adherence to regulatory standards.

Responsibilities

Requirements

• Valid nursing license (RN) in the appropriate jurisdiction.
• Strong clinical skills and knowledge of geriatric care.
• Ability to communicate effectively and empathetically with seniors.
• Attention to detail and ability to prioritize tasks.
• Ability to work collaboratively and adapt in a fast‑paced environment.
