St. Luke's Living Center West (LCW) is looking for an experienced Care Coordinator. LCW is located near downtown Cedar Rapids, Iowa. A skilled nursing facility with long lasting connections within the community of Cedar Rapids and surrounding areas. You will get to work with a great team of Nurses and training is provided! Essential Functions: Follow established standards, policies and procedures.
