Job Responsibilities • Evaluate and treat patients according to established standards of care • Coordinate specialty referrals, hospital/SNF transitions, DME, and home health services • Actively participate in daily team huddles • Support Lead Physician and Center Administrator in maintaining a cooperative work environment • Complete daily medical documentation with quality-based coder support • Follow clinical policies and protocols • Review quality outcomes and medical records with Lead Physician • Focus on direct patient care and related administrative duties Requirements • Graduate of accredited MD or DO program • Active and unrestricted state medical license (or willing to obtain) • Board Certified or Board Eligible (ABMS/AOA) in FM, IM, or Geriatrics • Active and unrestricted DEA license • Medicare & Medicaid Provider Numbers • Strong interpersonal and communication skills • Commitment to team-based care model • Willingness to practice in value-based care setting • Basic computer & EMR skills
