Physician Assistant (PA) or Nurse Practitioner (NP) Hybrid Role – In Office + Remote Alliance Medical Clinic Service Area: North Phoenix (Anthem) – West Valley – Scottsdale Alliance Medical Clinic and Ternion Physician Group are seeking a compassionate and motivated Physician Assistant (PA) or Nurse Practitioner (NP) to join our growing integrated care team. This is a hybrid clinical role combining in-office care with remote and on-site visits to assisted living communities, focused on the management of dementia, chronic illness, and palliative care needs in geriatric populations. Position Overview The PA or NP will provide comprehensive medical care to patients in: Outpatient office settings.
Assisted Living Facilities. Memory Care Communities. Group Homes.
Home-based or Telehealth visits (as appropriate). This role includes a mix of in-person visits and remote care coordination, supporting medically complex and cognitively impaired patients with a strong emphasis on: Dementia evaluation and management. Chronic disease management.
Goals of care discussions. Palliative-focused treatment planning. Medication reconciliation.
Care coordination with families and facility staff. Providers will participate in interdisciplinary team meetings, quality initiatives, and care conferences with facilities and outpatient teams. Schedule 4–5 days per week.
Combination of: In-office clinic days. Assisted living / memory care visits. Remote documentation or telehealth visits.
Primary Service Area Includes North Phoenix. Anthem. West Valley.
Scottsdale. Required Licensure / Certifications Active Arizona License as: Physician Assistant Nurse Practitioner Board Certification (Family, Internal Medicine, Adult-Gerontology preferred). Current BLS Certification.
DEA License. Personal Qualifications Compassionate and patient-focused. Comfortable managing geriatric and dementia populations.
Strong communication skills with families and caregivers. Self-motivated with ability to work both independently and within a team. Interest in palliative or supportive care models.
