General Notes Responsibilities Counsels and educates heart failure and transplant patients and their families/ caregivers concerning health conditions, treatment options and community resourcesDocuments delivery of cardiac critical health care and nursing processes in accordance with care standards and network nursing policyParticipants in orientation and education of staff and studentsObtains cardiac critical care patient history and performs assessment via observation, interview and examinationOrders, performs and interprets diagnostic studies for cardiac critical care patients. Performs health assessments, screening, and patient careProvides direct treatment and management of cardiac critical care conditions via prescriptive authority or referral to other healthcare providers or community resources. Engages in discharge planningOther related duties as assigned Required Qualifications Physician's Assistant or Licensed Registered Nurse credentialed from the Texas Board of NursingLicensed Advanced Practice Nurse Practitioner with Prescriptive Authority specializing in Cardiac Critical CareCredentialed from the Texas Board of Nursing in Acute Care nursing.
