Job Overview The Patient Care Coordinator Nurse (Junior Level) is a full-time position based in Mumbai, responsible for facilitating seamless patient care and communication between multidisciplinary teams and patients. Candidates must have a minimum of 1 year of relevant work experience and demonstrate a commitment to delivering high-quality nursing care in a fast-paced hospital environment. Qualifications and Skills Demonstrated excellence in patient safety, direct patient care, and comprehensive nursing skills (Mandatory skill).

Proven experience in direct patient care, ensuring patient comfort, safety, and compliance with healthcare standards (Mandatory skill). Strong command of foundational nursing practices and protocols to deliver evidence-based care in line with institutional guidelines (Mandatory skill). Effective soft skills such as communication, empathy, and teamwork to establish supportive relationships with patients and colleagues across departments.

Expertise in care coordination to streamline patient referrals, transitions of care, and documentation while reducing fragmentation in care delivery. Ability to provide clear, accurate, and compassionate patient education to promote understanding of treatment plans and informed decision-making. Competence in patient counseling, offering emotional support to patients and families throughout the continuum of care and aiding them in coping with diagnoses or procedures.

Talent for time management, prioritizing multiple clinical tasks, and maintaining accurate electronic health records and care plans for efficient workflow. Roles and Responsibilities Serve as the primary nursing contact for patients, addressing clinical queries, care needs, and coordinating appointments or follow-up services. Collaborate with multidisciplinary medical teams to implement and monitor individualized care plans tailored to patient needs across specialties.

Educate patients and their families regarding diagnoses, treatments, medications, and post-discharge instructions to ensure superior health outcomes. Support patients during admission, transfer, and discharge processes, coordinating with relevant departments to ensure smooth transitions. Prioritize patient safety initiatives by adhering strictly to infection control protocols, documentation guidelines, and institutional policies.

Conduct patient counseling sessions to provide reassurance, address concerns, and facilitate stress reduction during hospitalization. Monitor patient progress, promptly report changes in condition to the appropriate medical team, and advocate for their needs. Maintain meticulous patient records and facilitate seamless flow of information among healthcare professionals for coordinated and timely interventions.
