JOB RESPONSIBILIES • This position will act as a liaison to the Medication Therapy Management (MTM) program but will also work on transition care and other integrated care delivery needs. • Works in collaboration with the MTM program on outpatient medication management and utilization for MHS employees and families • Works closely with the ACO to help manage and develop preventative care, quality improvement and chronic disease management programs • Works with Hospitalist program and Primary Care physicians to develop, implement and manage transition of care management program • Reports directly to CMO QUALIFICATIONS • Board-certified Nurse Practitioner • Active Texas medical license • Minimum 2 years of experience in clinical practice • Strong interest in Medication Therapy Management • Ability to work effectively in a fast-paced, team-oriented environment
