We provide primary care services and palliative care consultations across assisted living facilities, subacute rehabilitation centers, long-term care settings, and patient homes throughout Camden, Burlington, and Gloucester counties. Position Overview We are seeking a kind, compassionate, and motivated Nurse Practitioner to join our growing team. This role focuses on providing primary care services and palliative care consultations to geriatric patients across a variety of care settings.
