Job Highlights Monday‑Friday 8:00 AM‑5:00 PM with on‑call hours every other weekend $130,000‑$145,000 annually 403(b) Retirement Plan Career scholarships Quality training, continuing career education and leadership programs Medical, Dental and Vision Insurance Paid Time Off (Vacation, Holiday & Sick Days) About the job The Nurse Practitioner, under the supervision of the Executive Director and with Medical Director oversight, provides primary care to participants. Responsibilities include performing physical assessments of new PACE enrollees, semi‑annual reassessments of participants, developing and implementing appropriate plans of care, evaluating participant physical complaints, providing treatment, and teaching participants and caregivers. As a member of the Interdisciplinary Team (IDT), the NP demonstrates the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to Senior Community Care standards. Required Qualifications Graduate of a school of professional nursing Licensed as Registered Nurse (R.N.) in the state of employment Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing Prescriptive authority within one year of hire Minimum of one year of experience working with the frail and elderly population Preferred Qualifications Certified as a Geriatric Nurse Practitioner (G.N.P.) preferred Two (2) years of experience as a nurse practitioner in a geriatric setting preferred Essentials Perform in‑person comprehensive history and physical assessments of new participants Coordinate with the Interdisciplinary Team to develop comprehensive plans of care Conduct in‑person reassessments semi‑annually and as needed Work with team members to meet emergent and acute medical and non‑medical needs Participate in discharge planning for participants in acute, skilled, and long‑term care settings Manage, evaluate, and treat participants with chronic diseases, maximizing interventions in the clinic or home before initiating higher level care Evaluate and treat participants with acute illnesses, maximizing interventions in the clinic or home before initiating higher level care Senior Community Care of Maryland - PACE PACE is a Program of All‑Inclusive Care for the Elderly.
