The Judge Group is looking for nurse practitioners throughout many counties in Washington to perform in-home health assessments on Medicaid/Medicare Advantage Members with one of the largest healthcare providers in The United States. Contract: Ongoing (No end date) Schedule: Extremely flexible schedule - Create your own part-time or full-time schedule Overview: During 45-90 minute in-home visits, nurse practitioners will conduct comprehensive health assessments. This includes reviewing medical history, medication adherence, and social determinants of health while observing the member's overall condition.

This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. Responsibilities: Conduct adult/geriatric assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to guidelines and protocols Work collaboratively with physicians, case managers, social workers, family members, key caregivers and any appropriate ancillary medical personnel as appropriate Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up Qualifications: Minimum of 6 months of experience as a nurse practitioner NP licensure Board certified by the AANP, ANCC in an approved specialty Have a current BLS, ACLS or CPR certification Have strong computer skills and familiarity with Employee health/medical record software Board Certifications accepted: Acute Care Adult Health Family Health If you are interested, please apply.
