Position Overview The Nurse Practitioner at Preferred Healthcare provides advanced clinical care, primarily focused on wound management and mobile patient services. This role combines direct patient care, patient education, and collaboration with interdisciplinary teams to deliver high-quality, compassionate care. The NP ensures patients receive individualized treatment plans and education, supporting recovery, prevention, and overall wellness in alignment with Preferred Healthcare’s mission and values.

Key Responsibilities Clinical Care & Patient Management Perform comprehensive patient assessments, including wound evaluation, medical history, and physical exams. Develop, implement, and manage individualized treatment plans for wound care patients. Provide direct patient care, including dressing changes, debridement, skin grafting, medication management, and education.

Monitor patient progress and adjust care plans as needed. Conduct mobile or in-clinic visits, ensuring high-quality care in any setting. Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and optimize outcomes.

Documentation & Compliance Maintain accurate, complete, and timely medical records in accordance with regulatory standards. Ensure compliance with HIPAA, state regulations, and Preferred Healthcare policies. Participate in quality improvement initiatives, clinical audits, and performance reviews.

Patient & Family Education Educate patients and caregivers on wound care management, prevention strategies, and self-care techniques. Promote patient adherence to treatment plans and encourage active engagement in care. Collaboration & Professional Development Serve as a clinical resource and mentor to other healthcare team members.

Stay current with clinical best practices, continuing education, and wound care advancements. Participate in clinical training and staff development initiatives. Qualifications Master’s degree in Nursing (MSN) from an accredited program.

Active and unrestricted Nurse Practitioner license in (state). National certification (e.g., AANP or ANCC) required. Minimum 2 years of NP experience; wound care, home health, or mobile healthcare experience preferred.

Strong clinical judgment, critical thinking, and patient assessment skills. Excellent interpersonal, communication, and patient education skills. Ability to work independently in mobile, field-based, or clinic environments while maintaining high-quality care standards.
