WCS Healthcare Partners is seeking a dedicated and experienced Infectious Disease Nurse Practitioner to deliver comprehensive primary care services to adult patients. The ideal candidate will be responsible for conducting health screenings, performing diagnostic evaluations, and implementing therapeutic interventions. This role requires collaboration with clinical staff to ensure coordinated care and excellence in service delivery.
Job Function:- Deliver primary care services to adult patients, including health screenings, diagnostic evaluations, and therapeutic interventions.
- Perform detailed medical assessments and formulate individualized care plans.
- Facilitate coordination of services and specialty referrals in partnership with clinical staff.
- Complete thorough and timely medical records documentation and engage in periodic chart audits.
- Work alongside clinical leadership to enhance care delivery processes and overall service excellence.
- Diagnose and manage infectious diseases, including implementing treatment plans and infection control measures.
- Provide patient education on prevention and management of infectious diseases.
- Collaborate with public health agencies to report and manage outbreaks of infectious diseases.
- New York State license, DEA, CSR preferred.
- Master's Degree as a Nurse Practitioner.
- 2 plus years of experience as a Nurse Practitioner.
