Position not covered by Agenda for Change We have an exciting opportunity for an enthusiastic experienced Practice Nurse to join our team. The role is 30 hours per week, days to include a Monday or a Friday The successful candidate should ideally be experienced in Primary Care with key skills to include management of all long term conditions, childhood immunisations and cervical cytology with a focus on delivery of the highest quality care to our patients. Main duties of the job Main Duties of the Role We are looking for someone with experience of working in busy primary care environment, who is both flexible and can adapt to the ever‑changing demands of primary care. The key candidate will want to deliver a holistic approach of care to patients.

The key skills and experience should include Team player Cytology Asthma, COPD and respiratory conditions Other Chronic disease management Childhood Immunisations A good understanding and or willingness to understand QOF and other enhanced services Any other roles and responsibilities of a traditional and forward‑thinking practice nurse. Excellent transport links, we are easy to reach from Manchester, Stockport, Altrincham and the surrounding areas, close to Manchester Airport, great local schools, lovely village and shops Job responsibilities To manage patients with chronic disease, run specific clinic sessions and maintain robust call and recall systems. Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule.

Provide travel health advice and administer appropriate vaccinations. The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence‑based practice for patients presenting with a long term condition, and the provision of preventative health care to the practice population.

As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision‑making in the management of patients. Participate in Quality Outcome Framework (QOF) and Extended Primary Care Service objectives Perform searches and audits Prepare for CQC inspections They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures. Treat To maintain own annual registration with the NMC and adhere to Code of Professional Conduct.

To maintain a safe and therapeutic environment for all PHCT staff, patients, carers and co‑workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc) are implemented and adhered to.

Any other reasonable duties that may be required from time‑to‑time. Qualifications Current NMC registration. Evidence of continued professional development Chronic disease management & knowledge of patients with long term conditions.

Contraception & sexual health. Cervical cytology. Experience Excellent communication & interpersonal skills.

Experience dealing with the public / patients. Good organisational skills. Knowledge of clinical governance.

Commitment to professional development Team player. Ability to organise own workload. Ability to work independently.

Understanding of confidentiality. Able to work under pressure. Able to use own initiative.

Flexibly & adapting to the management of change. Flexibility to cover annual leave and sickness. To work extended hours.

Understanding of current Primary Care issues. Computer skills or willingness to learn. Ideally EMIS & Microsoft Office.
