Job Description: As a Care Manager, you will play a crucial role in coordinating and overseeing comprehensive care plans for our clients. Your responsibilities will involve collaborating with healthcare providers, clients, and their families to ensure the delivery of high-quality, personalized care. You will be at the forefront of enhancing client well-being and satisfaction through effective management and communication.
