What You'll Do Deliver direct nursing care under the direction of a Registered Nurse (RN) in accordance with the plan of care Administer medications and treatments as ordered by the physician Monitor and report changes in the patient's condition and ensure timely communication with the RN and interdisciplinary team Provide education to patients and caregivers on self-care, medications, and symptom management Assist with daily living activities to promote patient comfort and independence Maintain accurate clinical documentation and timely reporting of patient care and progress Participate in interdisciplinary team meetings, quality improvement initiatives, and continuing education Ensure safe, respectful, and compassionate care at all times Qualifications Licensed Practical/Vocational Nurse (LPN/LVN) in the state of employment Graduate of an accredited Practical or Vocational Nursing program Minimum 1 year of nursing experience (hospice, home health, or related preferred) Active Basic Life Support (BLS/CPR) certification Valid driver's license, auto insurance, and reliable transportation Strong communication and time management skills Comfortable working independently and in rural or home settings Demonstrates compassion, empathy, and a deep understanding of end-of-life care Please note: Legacy Hospice is a drug-free workplace.
