Provide hands‑on care: assist clients with personal hygiene, mobility, light exercise, meals, housekeeping, errands, and medication reminders. Ensure quality & safety: complete home‑safety assessments, conduct routine quality‑assurance visits, and document plan‑of‑care updates for the Care Manager. Train & onboard caregivers: deliver one‑on‑one coaching, skills refreshers, and ongoing competency checks.
Serve on the on‑call rotation: triage caregiver call‑outs, fill urgent shifts, and resolve client concerns after hours. Caregiver Lead will be on call for five days from 8:00 AM to 8:00 PM. You should be ready to step in to provide hands-on care, guidance, and support for both clients and team members.
Problem‑solve in real time: address changes in client condition, escalate issues, and keep families informed. What You’ll Bring 2 years of professional caregiving, HHA, CNA, or home‑health experience (lead/supervisory duties a plus) Proven ability to coach peers and communicate effectively with clients, families, and clinical staff Reliable transportation and a valid driver’s license. Must have full knowledge of all types of care (bed-bound and dementia care) Tech‑savvy enough to document care notes in a mobile app or EMR Compassionate, ethical, and calm under pressure—professional in every scenario Ready to Lead with Heart?
