Primary Care Provider (Home-Based) Tennessee 3 month-Contract, possible extension Full-Time, Monday Friday, 8:00 AM 5:00 PM About the Role We are seeking a compassionate and experienced Primary Care Provider to deliver high-quality, patient-centered care directly in the homes of seniors. Key Responsibilities Conduct comprehensive in-home physical assessments and develop individualized care plans Provide ongoing management of chronic conditions and address urgent healthcare needs Deliver preventive care and routine primary care visits in a home-based setting Collaborate with physicians, nurses, and care coordinators to optimize patient outcomes Maintain accurate, timely documentation in compliance with regulatory standards Act as a patient advocate , educating seniors and their families on care plans and health management Required Qualifications 2+ years of clinical experience in primary care, geriatrics, or home health Active DEA license in Tennessee Currently enrolled in Medicare and Medicaid in Tennessee Preferred Qualifications Previous home health experience Why Join Us?
