Specialist Practitioner District Nurse (SPDN) – Job Purpose The District Nurse provides leadership for the delivery of patient care within their area of responsibility. They have day‑to‑day managerial responsibility for the area, including supervision and deployment of staff. The District Nurse will assess care needs, evaluate care plans, and ensure the delivery of high‑quality care to patients, while maintaining accountability for clinical and staff governance. Community Nurse Practitioner – Job Purpose The Community Nurse Practitioner utilises high levels of decision‑making and clinical judgement to meet patient health needs. They manage patient conditions through assessment, treatment, evaluation and review, ensuring the delivery of high‑quality person‑centred care in partnership with patients, families and carers.
The role also provides specialist advice to healthcare professionals, other agencies, carers, clients and relatives, encompassing specialist nursing and education. They provide day‑to‑day leadership for their caseload and allocated nursing team, including supervision and deployment of staff.
