Geriatrics Nurse Practitioner

The Nurse Practitioner (NP) plays a vital role in delivering high-quality, patient-centered primary care to older adults enrolled in a PACE (Program of All-Inclusive Care for the Elderly) model. This position provides care both in a clinic-based setting and, as needed, in participants homes or partner facilities. As a core member of the Interdisciplinary Team (IDT), the NP collaborates closely with physicians, social workers, therapists, dietitians, and other care team members to ensure seamless, coordinated care.

The NP is responsible for developing and managing individualized care plans that align with each participant s health goals, supporting independence, preventative care, and overall quality of life.
