Job Description Job Description We are a housecall company located in the DFW metroplex and we are seeking a Family Nurse Practitioner (FNP)to join our team, The Family Nurse Practitioner provides comprehensive healthcare across all ages, including adults and geriatrics in a home setting. This role involves conducting patient assessments, diagnosing and treating illnesses, and managing chronic conditions to promote optimal health outcomes. The FNP plays a crucial role in patient education, counseling, and coordinating care to ensure seamless healthcare delivery.

Responsibilities Assess patient health status through physical exams and history taking Diagnose acute and chronic medical conditions Develop and implement treatment plans Prescribe medications and monitor patient response Provide health education and counseling to patients and families Manage chronic disease prevention and management strategies Coordinate patient care among healthcare providers Monitor vital signs and document findings accurately Maintain comprehensive electronic health records (EHR) Preferred Qualifications Minimum 2 years of experience in family nursing Master’s degree in Nursing or related field Licensed Family Nurse Practitioner (FNP) Expertise in clinical assessment and pharmacology Strong patient communication and care management skills Proficient in electronic health records (EHR) systems Excellent critical thinking and time management abilities Demonstrates compassion and patient-centered care
