Advanced clinical care withexceptional patient services Job Description The FamilyNurse Practitioner (FNP) is an advanced practice registered nurse who providescomprehensive, evidence‑based primary and episodic care to patients across thelifespan in alignment with the mission, values, and clinical standards of EastHouston Medical Center (EHMC). The FNP functions as an integral member of amultidisciplinary team, promoting exceptional patient outcomes, serviceexcellence, and a culture of safety for the diverse East Houston community.

Key Responsibilities Deliverhigh‑quality primary and urgent care, including comprehensive histories,physical examinations, and age‑appropriate screenings for patients frominfancy through older adulthood. Diagnoseand manage acute and chronic health conditions (e.g., hypertension,diabetes, minor injuries, infections) and develop individualized, evidence‑basedtreatment plans. Prescribeand manage medications, durable medical equipment, and other therapeuticinterventions in accordance with Texas law, EHMC policies, and currentclinical guidelines.

Order,perform, and interpret appropriate laboratory tests, imaging studies, andother diagnostics; refine plans of care based on clinical findings andpatient response. Provideproactive health promotion, disease prevention, and risk‑reductioncounseling, including immunizations and routine screening programstailored to the East Houston population. Educatepatients and families on diagnoses, treatment options, self‑managementstrategies, and lifestyle modifications that support long‑term wellnessand health equity.

Coordinatecare and make timely referrals to EHMC specialists, ancillary services,and community resources to optimize continuity of care and patientexperience. Maintainmeticulous, timely, and compliant documentation in the electronic healthrecord, reflecting clinical reasoning, interventions, and patientresponse. Collaborateclosely with physicians, nurses, case managers, and allied healthprofessionals to support integrated care pathways, throughput, and qualitymetrics across EHMC.

Upholdand model EHMC’s standards for patient safety, infection prevention,regulatory compliance, and service excellence in every patient and familyencounter. Current licensures as aRegistered Nurse (RN) and Advanced Practice Registered Nurse (APRN) in theState of Texas (or eligibility to obtain). National certificationas a Family Nurse Practitioner (FNP‑C or FNP‑BC) by a recognizedcertifying body (e.g., AANP or ANCC). Current Basic LifeSupport (BLS) and Advanced Cardiovascular Life Support (ACLS)certifications; additional life support credentials as required by EHMCclinical area.

Minimumof one to three years of recent FNP or advanced practice experience in aprimary care, ambulatory, or acute‑care setting preferred; new graduateswith exceptional clinical performance may be considered.
