Description Conduct patient evaluations in inpatient and outpatient contexts, customizing treatment plans to meet unique needs. Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functional ability. Coordinate with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimized patient outcomes.
Guide patients and their families through therapeutic goals, discharge planning, and at-home exercise regimens. Maintain detailed documentation of evaluations, treatments, and patient progress following facility policies and regulatory standards. Engage in care team meetings and interdisciplinary discussions to ensure seamless patient care.
Education A Doctorate in Physical Therapy (DPT) or a related accredited degree. Certifications License to practice as a Physical Therapist in the state. CPR certification is preferred.
Desirable specialized certifications (e.g., neurological or geriatric rehabilitation) are welcomed but not essential. Skills Prior experience in a clinical setting is highly preferred. Adeptness in treating patients with intricate medical and mobility concerns.
