Director Of Patient Services

Location: Brooklyn/Manhattan Salary: $120K-$145K We are seeking an experienced and compassionate Director of Patient Services (DPS) to oversee and manage the clinical operations of our home care agency located in Brooklyn. The DPS will ensure that all patient care services meet the highest standards of quality, safety, and compliance with New York State Department of Health (DOH) regulations. The ideal candidate is a Registered Professional Nurse (RN) with strong leadership, communication, and organizational skills, and a deep commitment to improving patient outcomes in the home care setting.

Responsibilities for Director of Patient Services (DPS): Oversee the delivery of home care services to ensure compliance with DOH regulations, agency policies, and professional standards. Supervise and support nursing staff, home health aides, and other clinical personnel. Conduct patient assessments and develop individualized care plans in coordination with the clinical team.

Ensure accurate and timely documentation of all patient services and clinical activities. Collaborate with physicians, caregivers, and families to promote continuity of care. Lead quality assurance and performance improvement initiatives.

Train and mentor clinical staff on agency procedures, regulatory compliance, and best practices. Participate in surveys, audits, and inspections conducted by regulatory agencies. Maintain up-to-date knowledge of home care regulations and ensure agency readiness at all times.

Director of Patient Services (DPS) MUST have an RN license in the state of NY
