Community Nurse Practitioner: Lead Clinician & Educator

Specialist Practitioner District Nurse (SPDN) – Job Purpose The District Nurse provides leadership for the delivery of patient care within the area of responsibility. The District Nurse has day‑to‑day managerial responsibility for the area, including the supervision and deployment of staff.

The District Nurse will assess care needs, evaluate care plans and ensure the provision of high‑quality care to patients. The post holder is responsible and accountable for maintaining both clinical and staff governance. Community Nurse Practitioner – Job Purpose The Community Nurse Practitioner utilises high levels of decision making and clinical judgment to meet patient health needs through the assessment, treatment, evaluation and review of plans, ensuring the delivery of high‑quality person‑centred care to patients in partnership with patients, families and carers.

The role provides specialist advice to healthcare professionals, other agencies, carers, clients and relatives. This role encompasses that of specialist nurse and educator. The Community Nurse Practitioner provides day‑to‑day leadership for a defined caseload and allocated nursing team, including supervision and deployment of staff.
